The Company accepts your resignation and in accordance with your contract of employment you are required to work your notice period.  You will be paid up to and including [date] when your employment will end. Your P45 will be issued to you shortly thereafter.  
According to our records you have [number] days remaining holiday entitlement to the effective date of termination.  [The Company requires you to take your remaining holiday entitlement during your notice period and therefore your last working day will be [date].  ]
