Asking your employer for an annual leave is a simple process. You just have to keep few things in your mind like the tone of the letter should be formal, the letter should be concise with the exact number of days you will be on leave. It should also mention the date of joining once you will be back from vacations. It will be a good thing if you will give a complete status of your job work as it will help your employer to understand your schedule and if needed he can hand over your work to other members of your team.
Here is a sample of annual leave letter:

Dear Ms. Nomes,
Ref: Annual Leave Letter
I am writing this letter to request you to formalize my annual leave of four weeks that I can avail as part of my employment with the organization. I wish to avail my leave from 21st May, 2010.
The reason for this is that I am getting married on 25th August, 2010. So I need some vacation to set and start a new dimension of life. As a senior quality analyst I understand that I have the responsibilities of three leading projects and I want to assure you that before going on leave I will complete the in hand projects. I also assure you that my team members will handle rest of the things in my absence and I will be in regular contact with them to help them at any level.
I will take the charge from 20th June, 2010 and if there will be any delay in joining then I will confirm that a week before.
Thank you and hoping for your kind consideration.
Sincerely,
Alex Simond
